New Patient Forms

You can use the link below to access our patient forms. If you are able to, please download the forms, print them out, and complete them before you come in for your first visit to save time. If not, you are more than welcome to fill them out in our office.

Preparing for Your First Visit

Here’s a helpful checklist of how to prepare for your visit:

Bring the following to your appointment:

  • Your driver’s license
  • Your current insurance card (health, and for car accidents, auto)
  • Any pertinent medical records, any diagnostics or testing (like an X-ray or MRI) that you may have taken elsewhere, the names and contact information for any other health care providers you have seen
  • Any attorney information
  • Any other documentation on your ailment, injury, or accident
  • Your completed New Patient Forms (if you were able to fill them out beforehand)

Wear something comfortable, like jeans, sweat pants, shorts, and a T-shirt. Come in minimal (if any) jewelry.

What to Expect

Your evaluation may take between 30 minutes and an hour, depending on your situation. Your initial treatment time may take another 20 to 45 minutes after that, if you do receive treatment on your first visit. We will make every attempt to treat you on this first day; however, it depends on your injuries, conditions, and whether further diagnostic testing is needed. The priority is to have an accurate diagnosis, prognosis, and treatment plan first before we begin treating you.

During this initial evaluation and exam, we first will go over your medical history and current condition. Next, we will conduct a physical, orthopedic, and a neurological exam. Then (depending on these first two steps), we will take X-rays if required. After that, we review the findings of this assessment and discuss with you if further testing or referrals are necessary, or if you can begin treatment (and what that treatment would involve). Time permitting, we will then get to work helping you to feel better!
